You may fill out most of the fields on-line but you may need to fill in the party name in Section B by hand, as well as providing your handwritten signature. Having done so, please scan it, and return it to the following email address: jonescp36@gmail.com. Alternatively, you can simply print the blank form and fill it in entirely by hand, and then sign it, scan it, and email it back to us. Thank you
